USB 1.1 spec - Input voltage

Hello,
Looking through the USB 1.1 specification (section 7.3.2 Electrical characteristics) there are two values listed for the high voltage, driven and floating. Could anyone explain the difference between the two? Looking at some V-USB examples it appears the the minimum value expected for a high value is 3.3V, is this value statistically gathered (through real-world tests) or is it mentioned explicitly somewhere else in the specification?
